For existing AIM to Prevent program members, the best way to stay engaged with your ongoing health and wellness is to participate in the annual membership after competion of the program's first year.

Existing AIM to Prevent participants will receive these benefits on an ongoing basis for another year:

  1. Continued coaching with your personal Heart Health Coach: Monthly for 1 year OR bi-monthly for 6 months as per your preference.
  2. Clinical risk profile re-evaluation: We update your comprehensive risk evaluation report and recommendations based on recent assessments. 
  3. Access to lifestyle resources, workshops, huddles, and other health events as scheduled during your membership year. 

We look forward to having you stay engaged on your wellness journey with us.

This membership is only available to those who have completed the AIM to Prevent program within the last 10 years.
