87738 - AIM to Prevent Yearly Wellness Checkup
with South Asian Heart Center Staff
For existing AIM to Prevent program members, the best way to stay engaged with your ongoing health and wellness is to participate in the annual membership after competion of the program's first year.
Existing AIM to Prevent participants will receive these benefits on an ongoing basis for another year:
- Continued coaching with your personal Heart Health Coach: Monthly for 1 year OR bi-monthly for 6 months as per your preference.
- Clinical risk profile re-evaluation: We update your comprehensive risk evaluation report and recommendations based on recent assessments.
- Access to lifestyle resources, workshops, huddles, and other health events as scheduled during your membership year.
We look forward to having you stay engaged on your wellness journey with us.
This membership is only available to those who have completed the AIM to Prevent program within the last 10 years.
87738 - Insights with Real-Time Blood Sugar Monitoring
with South Asian Heart Center Staff
Ever wonder about the impact of your lifestyle on your blood sugar levels?
What happens to your blood sugar when you eat? Or exercise? Or are stressed?
Would you like to uncover your hidden glucose patterns and get a complete picture of where your glucose has been and where it’s going?
Now you can! With the advent of continuous glucose monitoring, you are able to correlate your lifestyle habits with the efficiency of blood sugar regulation in your body in real-time.
Join this two week program to observe your blood sugar levels on a continuous basis. You may even get to experiment and understand how tweaking your lifestyle may affect these levels.
You are eligible to register if you meet the following criteria:
- You are a first time CGM (Continuous Glucose Monitor) user
- You are not currently diagnosed with Type-1 diabetes
- You are not currently pregnant
- You have no prior history of hypoglycemia
- You have a healthcare provider

87738 - STOP-D™: Stop Diabetes Before it Starts

STOP-D is an engaging yearlong, CDC Full-Plus Recognized National Diabetes Prevention Program, aimed at losing body weight in a sustainable way to lower blood sugars and prevent and/or better manage diabetes. This high-touch, comprehensive and structured program, includes interactive zoom classes, your own personalized lifestyle coach, meetings with a registered dietitian, group support, and much more!
STOP-D includes:
- Comprehensive clinical and lifestyle risk assessment
- Detailed recommendations based on your profile
- 22+ interactive zoom sessions covering the National Diabetes Prevention Program curriculum
- 4+ evidence-based workshops on Lifestyle MEDS (Meditation,Exercise,DietandSleep)
- Frequentpersonalcoaching sessions
- Group/social support and weekly motivations
Baby Proofing (Online)
available
We no longer offer baby proofing classes, but they are available at Stanford Medicine Children's Health (stanfordchildrens.org)
Car Seat Check (In-person)
available
A nationally certified car seat technician (CPST) will visually inspect your child’s car seat. Recommended to be taken in the 7th month to the early 9th month of pregnancy.

Community Health Huddle

El Camino Health presents the Thursday Evening Community Health Huddle (TECHH), a monthly community virtual health series for you and your friends to participate in from the comfort of your home. Join our faculty of physicians and wellness experts to discuss strategies with a new health topic each month.
Ask questions, hear others, and feel inspired to work on our health and the health of those we influence.

78820: Breastfeeding Online Support Group - Virtual Session
with Lactation Consultant
Provides support for mothers and their support person(s) on topics such as breastfeeding, latching, nipple soreness, low milk supply, newborn fussiness and sleep, baby weight gain, pumping and much more. Led by an International Board Certified Lactation Consultant (IBCLC). This 1½-hour virtual group session meets every Thursday from 10:30 am to 12:00 pm. Session is free and recommended for mothers of newborns through 6 months of age.
One (1) registration per household
Registration required at least 24 hours in advance to ensure access to the class.
